Before we begin, it's worth noting that there are two ways to create forms in Google Docs. Start a form

To create a form from Google Sheets, go ahead and click the Insert tab and then click Form.

You will notice that when you return to your spreadsheet, you will see a message that a new form has been created. Additionally, if you click on the Form tab, you will be able to edit the form, submit it, view the live form, and so on.
